The Registrant desires to participate in the following Program activity (the “Program”) which is sponsored by Uplands Reach Conference Center, Inc. (“URCC”) as a Conference Center/Camp Activity. Camp activities may include, but are not limited to, athletic activities, tubing, canoeing, kayaking, archery, shooting, wall climbing, horseback riding, aquatics, hiking, low ropes course, wilderness skills training, and overnight camping.
By attendance, you, the Registrant (or Parent/Guardian of Registrant) agree to comply with all URCC rules and regulations for this event. By initialing below, "I agree to the terms above: I agree to abide by all rules, policies, and procedures adopted by URCC. This could possibly include URCC's Summer Team guidelines and if Registrant is 18-years-old or older has completed the online background check (Link provided by URCC).
The Registrant (or Parent/Guardian of Registrant) acknowledges that
(i) his/her participation is voluntary;
(ii) he/she has been advised that the Program may not insure against certain kinds of risks related to the Registrant's participation or his/her child’s participation in the Program and that the policies of insurance maintained by URCC may not provide coverage in the event of illness, accidental injury, or death resulting from participation in the Program;
(iii) he/she has a current policy of health insurance which provides adequate coverage or otherwise accepts full financial responsibility in the event the Registrant or his/her child becomes ill or injured in any way as a result of his/her participation in the Program;
(iv) he/she understands and acknowledges that participation in the Program necessarily involves the risk of injury and/or property damage or loss, and that by checking the box below and by participating in the Program he/she specifically and knowingly assumes any and all risk(s) of loss or injury, including, but not limited to illness, emotional injury or distress, serious injury or death. The Registrant (or Parent/Guardian of Registrant) further hereby consents to the rendering of any medical treatment that may be necessary or appropriate in the event that the Participant is injured and is unable to consent to medical treatment at the time of the injury or at anytime thereafter when medical treatment is appropriate or necessary.
The Registrant (or Parent/Guardian of Registrant) acknowledges that he/she or his/her child may require transportation while participating in the URCC Program. He/she acknowledges that said transportation may consist of vans owned by URCC and operated by URCC personnel, by vans/buses with operators provided by the church/group with whom he/she or his/her child is attending the Program, or by vans rented by URCC and operated by URCC approved personnel. The Registrant further hereby consents to the transportation of himself/herself or his/her child by URCC during the Program, and he/she assumes any and all liability for damages to or caused by his/her child in connection with the transportation services provided by URCC.
This event will be documented by URCC through video, audio, and photographic media. Most likely, attendees will be filmed, recorded, or photographed within a group or individually. By attendance, you agree to said documentation and grant permission for URCC to utilize your likeness, words, or voice for commercial or internal use without compensation, specifically waving rights of privacy during the filming, recording, or photographing, and releasing URCC from liability for loss or damage. By initialing the "I agree to the terms above" and by his/her participation or his/her child’s participation in the Program, the Registrant consents to the video, audio, and photographic documentation that will take place during the Program, and gives permission for URCC to utilize his/her or his/her child’s likeness, words, or voice for commercial or internal use without compensation, specifically waiving rights of privacy during the filming, recording, or photographing, and releasing URCC from liability for loss or damage.
Furthermore, by initialing the "I agree to the the terms above" and participating in the Program, the Registrant with the intention of binding himself/herself, and his/her heirs, legal representatives and assigns, expressly releases and discharges URCC, its directors, officers, sponsors, and employees from any and all claims, demands, actions, judgments, and executions that the Registrant may have which are created by or arise out of his/her participation or his/her child’s participation in the Program. The Registrant also agrees that he/she will not bring or prosecute or allow to be brought or prosecuted any claim or suit for damages or any other relief in any way related to his/her participation or his/her child’s participation in the Program and agrees to indemnify and hold harmless URCC, its directors, officers, sponsors, and employees from any such claim of liability or damage, including, but not limited to, any and all legal fees related to the URCC’s defense of any such claim of liability or damage.
By initialing the "I agree to the terms above", I, the Registrant (or Parent/Guardian of Registrant), certify that I have read the waiver above in its entirety and agree to be bound by the terms and conditions stated therein. The medical information, if any, provided above is accurate and complete to the best of my knowledge.
By initialing the "I agree to the terms above", I, the Registrant (or Parent/Guardian of Registrant), agrees to the truths and teachings of the Baptist Faith and Message 2000,(http://www.sbc.net/bfm2000/bfm2000.asp) that will be taught at this event by Uplands Reach.
By initialing the “I agree to the terms above” in the box below, I am signing this agreement electronically. I agree that my electronic signature is the legal equivalent of my manual signature. By selecting “I agree to the above waiver” I consent to be legally bound by this agreement’s terms and conditions. I further agree that my use of a key pad, mouse, or other device to select an item, button, icon or similar act/action, or to otherwise provide Uplands Reach Conference Center, Inc. (“URCC”) instructions via its online portal or in accessing or making any transaction regarding any agreement, acknowledgement, consent terms, disclosures or conditions constitutes my signature (hereafter referring to as “E-Signature”), acceptance and agreement as if actually signed by me in writing. I also agree that the validity of my E-Signature shall be determined under the laws of the State of North Carolina and that no certification authority or other third party verification is necessary to validate my E-Signature and that the lack of such certification or third party verification will not in any way affect the enforce-ability of my E-Signature or any resulting contract between me and URCC. I also represent that I am authorized to enter into and or submit this agreement for all persons and or all persons in my household that are members or participants in a URCC program and that such person(s) will be bound by the terms of this Agreement.
